AMATEUR ATHLETIC MINOR WAIVER AND RELEASE OF LIABILITY
In consideration of being allowed to participate in the Judson Jr. Rockets (JJR) athletic/sports program, Texas Youth Football Association (TYFA) & Cheer and related events and activities, the undersigned:
The risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
FOR MYSELF, SPOUSE, AND CHILD, I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my child's participation; and,
1. AGREES that the parent(s), and/or legal guardian(s) will instruct the minor participant that prior to participating he or she should inspect the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she should immediately advise his or her coach or supervisor of such condition and refuse to participate.
2. ACKNOWLEDGES and fully understands that each participant will be engaging in activities that involve risk of serious injury, including permanent disability or death, and severe social and economic losses which might result not only from their own actions, inaction, or negligence, but the action, inaction or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, that there may be other risks not known to us or not reasonably foreseeable at this time.
3. ASSUMES all the forgoing risk and accepts personal responsibility for the damages following such injury, permanent disability, or death.
4. RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE the Judson Junior Rockets, TYFA & Cheer, its affiliated clubs, their respective administrators, directors, agents, coaches, and other employees of the organization, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and leasers of premises used to conduct the event, all of which hereinafter referred to as “releases”, from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.
Insurance Waiver
I, the Parent/Guardian of this child, do not have my own Health/Injury insurance coverage. I assume all risks and hazards incidental to such participation without Health/Injury insurance coverage and do hereby waive, release, absolve, indemnity, and agree to hold harmless, the JJR, TYFA & Cheer, the host organization, the sponsors, supervisors, participants, volunteers, and any other persons involved in the JJR, TYFA & Cheer.
I, the Parent/Guardian of this child, believe to the best of my knowledge that he/she can withstand the rigors of a football or cheerleading season. I further believe there is nothing physically or mentally wrong with my child. I hereby approve of my child participating in the upcoming season without a physical examination, which is recommended by the JJR, TYFA & Cheer. I assume all risk incidental to such participation and do hereby waive, release, absolve, indemnify, and agree to hold harmless, the JJR, TYFA, the host organization, the sponsors, supervisors, participants, volunteers and any other persons involved in the JJR, TYFA & Cheer.
Deposit for Football Equipment Policy
I agree to and understand all of the following: tackle football equipment issued to my child is and remains the property of the JJR and I agree to return all equipment (Helmet & Shoulder pads) upon request by an authorized representative of the JJR Organization.
The check will be returned to me the same day my child’s equipment is returned; if I fail to return the equipment by 3rd week of December, the Judson Jr. Rockets Organization will cash my check on the Monday of the 3rd week of December, of the current year.
I will make the check good or funds available; not to stop payment, change accounts, or deny payment of the deposit check. I assume all legal responsibilities and fees incurred in recovering the equipment or funds from any check that is not made good if cashed or processed.
The Flag division does not require a deposit.
Payment/Refund Policy
Spring 2024 Registration Fees: Flag $105 / Tackle $135.
A child is not considered registered until full payment is received and cleared by the JJR banking institution. Payment Plans are available.
A $30 fee will be assessed for any check returned to the JJR.
Refund requests must be made, to the JJR Board of Directors, on or before, the 6th scheduled practice (two weeks) from the date of signing this document. There will be NO REFUNDS after that time; whether attending practice or not.
All refund requests are treated on a case-by-case basis. If an exception to this policy is granted, a partial refund will be provided.

No refunds will be provided if your child quits, becomes unable to participate, or is removed from the organization for disciplinary reasons.
Fundraising Events
The Judson Jr. Rockets Youth Sports Organization conducts 1 major fundraiser during EACH season to help pay for our end-of-year Sports Banquet, and trophies, as well as to offset the cost of uniforms and equipment. Every dollar that comes into the program is spent on the program.
We require 100% participation from all cheerleaders, football players, and parents to meet our fundraising objective.
You may choose to Opted Out of the Fundraiser by paying the Opted Out Fee at the time of registration. Please choose the Opted Outed Option below under Fundraiser if you wish to Opted Out.
Please indicate that as the Parent/Guardian of the child listed in the name box above, you have read and understand the statement on our fundraising policy and you will be participating by entering your Full Name in the box below.
Restricted Use of Name and Logo
I agree that the name, Judson Jr. Rockets, and the Logo are the sole property of the Judson Jr. Rockets Organization and that I will not use, incorporate into, or display in any manner without the expressed WRITTEN consent of the Board of Directors. I agree that the Judson Jr. Rockets Organization, Board of Directors, have sole jurisdiction in determining if a violation of this policy occurs and that they have sole authority for determining all sanctions against me including suspension or expulsion of myself, my family, and my child(ren) and that I forfeit all fees, funds, or other items given to me by the Judson Jr. Rockets Organization.
Media Release
I hereby give my permission for the release of photographs, videos, digital/audio recordings of the images, and likenesses taken while my child is participating in all JJR Football & Cheerleading sponsored activities.
